Free Model Release Forms

Below are two release forms that you, as a model or photographer, will need and use often. Please feel free to download them and make as many copies as you need.

Standard Model Release Form(Adobe PDF)

This is your standard model release form. Be sure to read each section and understand it. Most photographers have their own Model Release Form but you CAN ask to use yours.

 There are no set rules as to when you do or don’t need a model release, but it's important you protect yourself and your work legally. If you're going to sell or use an image of a for commercial purpose, you need to use a Model Release Form. It shows that you have permission to use the images, and that the subject has willingly handed over the usage rights to the photographer.
